A Voting Provider Member must be a Pennsylvania organization, have a direct
relationship with the Office of Long Term Living (OLTL) and if the consumer-employer model is provided, a separate EIN must
be used for these responsibilities.
A Non-voting Associate Member must be a Pennsylvania disability advocacy organization
or an association that is not itself a service provider.
